ReActiv8® Neurostimulation

Chronic low back pain can be life-limiting — especially when it persists despite physical therapy, medications, injections, or other conservative treatments. For many patients, the underlying cause is mechanical low back pain related to dysfunction of the multifidus muscle, an important stabilizing muscle of the spine.

ReActiv8® Restorative Neurostimulation, developed by Mainstay Medical, is an FDA-approved treatment designed specifically for patients with chronic mechanical low back pain who have not found lasting relief with traditional approaches and are not good candidates for spine surgery.

What Is ReActiv8?

ReActiv8 is an implantable restorative neurostimulation system that targets the root cause of certain types of chronic low back pain. Unlike spinal cord stimulators that aim to block pain signals, ReActiv8 is designed to restore proper muscle control and spinal stability by reactivating the multifidus muscle.

The system consists of:

  • A small implantable pulse generator
  • Thin leads placed near the nerves that control the multifidus muscle
  • An external controller that allows you to activate therapy sessions at home

The pulse generator emits mild electrical pulses, which are carried by the leads to the specific nerves that control the multifidus muscle. The electric pulses activate the nerves, causing the muscles to gently contract. Over time, this helps to rebuild the muscles, restoring spinal support and relieving pain. ReActiv8 is intended to be used for two 30-minute sessions each day.

Who is a candidate for ReActiv8?

ReActiv8 may be an option if you:

  • Have chronic low back pain lasting longer than six months
  • Have not achieved lasting relief from conservative treatments
  • Do not have an indication for spine surgery
  • Have pain related to mechanical instability rather than nerve compression

A comprehensive evaluation at Twin Cities Pain Clinic is required to determine if ReActiv8 is appropriate for your condition.

What to expect during the procedure

The ReActiv8 implant procedure is typically performed as an outpatient or same-day surgical procedure.

During the procedure:

  • You will receive anesthesia for comfort
  • Your physician will place the leads near the appropriate spinal nerves
  • The small pulse generator is implanted under the skin, usually in the lower back or buttock area
  • The incisions are closed, and you will be brought to a recovery area.

You will be monitored in the recovery area for a short time while your anesthesia wears off. The surgical team will review your postoperative care instructions and then you will be able to go home.

What to expect after the procedure

After implantation, your care team will program the device and teach you how to use the external controller.

Recovery and therapy expectations include:

  • Mild soreness at the incision sites for several days
  • Gradual return to normal activities as directed by your physician
  • Performing two 30-minute therapy sessions per day at home
  • No reliance on paresthesia (tingling sensations) during treatment

Improvement is progressive, with many patients noticing meaningful benefits over several months as muscle function and spinal stability improve.
