To make sure your visit goes smoothly, we have created a list of items to help you prepare for your appointment.

Save time by completing paperwork in advance and bringing the appropriate items to your visit.

What You Need to Do

Please take note of the following:

  • Your appointment time is set aside specifically for you. If you are unable to keep an appointment, you must provide us with a 24-hour notice. Failure to do so will result in a $50.00 cancellation fee. This fee is not covered by insurance. This fee must be paid before you are allowed to schedule another appointment. Repeated failure to meet this requirement may result in discontinuation of care at our clinic.
  • You will not be prescribed medications at your first visit so please plan accordingly.

Please bring the following with you to your appointment:

  • A valid driver’s license or other form of photo identification
  • Your current insurance card(s)
  • Your copay, if you have one – COPAYS ARE DUE DAY OF SERVICE
  • A list of medications you are currently taking and the original bottles so we can see the dosage, prescriber, and pharmacy
  • Any medical records and/or imaging reports related to your condition

What you can expect

  • Please arrive at least 15 minutes before your appointment so we can check you in and collect additional paperwork if needed.
  • A member of our clinical staff will show you to an exam room, where they will take your vitals, including blood pressure and weight, review your current medications, and discuss the reason for your visit.
  • Next, you will meet with your provider, who will conduct a brief examination to better understand the cause of your pain and begin a treatment plan. The provider will usually be accompanied by a scribe, who is an assistant that keeps detailed records of the exam.
  • After your exam ends, our front desk staff will schedule your next appointment and go over the next steps in your care.
  • If you were referred to Twin Cities Pain Clinic, the referring provider will receive copies of your appointment notes.

  • A member of our clinical staff will show you to an exam room where they will take your vitals, including blood pressure and weight, review your current medications, and discuss the reason for your visit.
  • Next, you will meet with your provider to check your pain levels, review your care plan/effectiveness of treatment, and discuss next steps.
  • When your visit has concluded, our front desk staff will help you schedule your next appointment and go over any remaining details.
  • If you were referred to Twin Cities Pain Clinic, the referring provider will receive copies of your appointment notes.

A Few More Things

Please respect our other patients’ time by giving at least 24 hours’ notice to cancel or reschedule an appointment. If you miss an appointment or cancel more than two appointments less than 24 hours in advance, we reserve the right to discontinue your care at our clinic.
