Prepare for Your Visit

We want your experience with Twin Cities Pain Clinic to be exceptional in every way.

To make sure your visit goes smoothly, we have created a list of items you help you prepare for your appointment.

You will be able to save a lot of time at the clinic by completing the required paperwork in advance and bringing the appropriate items to your visit.

What You Need to Do

Complete the New Patient Intake Form

  • Click here for the printable version of the form. You will need to bring this completed form to your appointment.

Please bring the following with you to your appointment:

  • A valid driver’s license or other form of photo identification
  • Your current insurance card(s)
  • Your copay, if you have one, which is due at time of service
  • A list of medications you are currently taking as well as the original bottles so we can see the dosage, the prescriber, and the pharmacy
  • Any relevant medical records and/or imaging reports

What you can expect

You will be required to wear a face mask that covers your nose and mouth at all times while in the clinic. We can provide you with a mask if needed.

Do not bring anyone into the clinic with you unless medically necessary.

Please practice safe social distancing while in the facility.

 

Plan to arrive at least 15 minutes before your appointment time so we can get you checked in and collect any additional paperwork if necessary.

A member of our clinical staff will show you to an exam room, where they will take your vitals, including blood pressure and weight. They will then review your current medications and discuss the reason for your visit.

Next, you will meet with a nurse practitioner or physician assistant, who will further review the details of your pain and conduct a physical examination. This will allow them to better understand the cause of your pain and begin a treatment plan. The provider will usually be accompanied by a scribe, who is an assistant that keeps detailed notes and records of the exam.

Depending on the recommended course of treatment, you may also meet briefly with a physician to discuss any final details and complete the care plan.

You will then return to the reception desk, where a member of our patient services team will help you schedule your next appointment and go over the next steps in your care.

If you were referred to Twin Cities Pain Clinic, the referring provider will receive copies of your appointment notes.

A Few More Things

Please note, you will not be prescribed medications at your first visit, so plan accordingly.

Please respect our other patients’ time by giving at least 24 hours’ notice to cancel or reschedule an appointment. If you miss an appointment or cancel more than two appointments less than 24 hours in advance, we reserve the right to discontinue your care at our clinic.
